Taylah Levett, 15, was last seen leaving a house at Yamba Rd, Yamba, about 7.45pm on Tuesday, October 5.
She is known to frequent Casino, Lismore and Yamba areas.
Her family and police are concerned about where Taylah is because she is so young.
Taylah is described as being of Caucasian appearance, about 160-165cm tall, of slim build, with long brown hair and brown eyes. She was last seen wearing a maroon-coloured zipped jacket and chequered track pants.
